Please use the form below to report your intermediate results to the foundation.

Additional documents

At the end of the interim report form you have the possibility to upload photos or additional documentation (financials, a short summary of your own design etc.). Limit your short summary, if any, to 2-4 pages.

About this online form

You can download a blank copy of the form to review in advance but only an online submission is accepted.

The «Save and continue later» button at the bottom allows you to save the form and change or add to it later. It generates a link which is valid for 30 days after the last save and can be forwarded by e-mail.

A form can be saved multiple times before submitting; the link stays the same.

The link can be shared between people but the form cannot be edited or saved by two people at the same time! Please coordinate within your team to avoid lost work.

You can review the form draft and download it as a PDF.

The document has a «Draft» watermark, as the form has to be submitted online using the «Submit» button.

Please note that the draft is not saved – you must use the «Save and continue later» button.

After submitting the form, an e-mail is sent with a PDF copy of your report.

Is the project on track in terms of content and time? If not: please give reasons.

Beneficiaries and participation

Which and how many beneficiaries have participated in your project so far?

Challenges, opportunities and risks

Were there delays, additional, unplanned results or changes in the environment? Have risks listed in the application materialized or do they still exist? Have unexpected opportunities arisen?

Comprehensive sustainability in the reporting period

During the reporting period, how has the project changed the environment and the social and economic situation of the beneficiaries/society?

Review and outlook

What are the highlights from the reporting period and what are the «lessons learned»? What are the important goals/milestones/methods and measures for the next reporting period?

Financials

Please compare budget and actual costs in the reporting period and explain significant differences. Budget/costs refer to all sub-projects (do not just submit the part contributed by the Foundation for the Third Millennium).
